This blog post by HAYWARD ZWERLING, MD seem to be a good start. He developed an EMR that was in use from 1990 to 2015.

Well, he got that part right. The rest is related to a specific context, and deals too much with data entry vs. disease management workflow.

Observation: you need to be able to see all features, and in context (unlike this forum where context is mixed). Everything else should be centered around the workflow: INTAKE ASSESSMENT PLAN INTERVENTIONS OUTCOME REVIEW REASSESSMENT or DISCHARGE
